SD District 535 Provides Opt Out Military Recruitment Form

Under the Federal No Child Left Behind legislation military recruiters may request student information from school districts.  Typically the military recruiters go to the district registration and records department in Edison at the end of September.  They request the mailing labels for all students in the junior and senior grades.  The mailing labels have home addresses and phone numbers.

However, there is provided by law an opt out form which parents may use to request that this information not be released to the military.

This is Form 515A2 NOTICE OF RIGHT TO REFUSE RELEASE OF PUBLIC DATA TO MILITARY RECRUITERS.  It may be obtained from the office of Registration and Records at Edison.  It may also be downloaded from the district website.  Go to the district home page and select RESOURCES from the top headings.  Then go to FORMS and the form will be found under the Dept. of Registration and Records.

A direct link to the form can be found at:

The form must be signed by a parent or guardian unless the student is 18 years of age.  It must be returned to Edison before the military requests the list.  A new form must be completed annually.  This process is explained in the district “Rights, Rules Regulations & Procedural Code” (page 21 of the 2005-06 handbook).

If the form has been completed and returned then the student’s name will appear on the mailing lists but no other information will be given to the military recruiters.  The district has a second form for refusal to release ANY information.  It is NOT necessary to complete this form to refuse information only from military recruiters.

Please contact the Rochester school district office of registration and records at the Edison building (285-8549) if you have any further questions.
